APPLY FOR TAX-EXEMPTION

Do you have a tax-exemption or reseller certificate?

In order to fully activate your account with C2G, we ask that you provide your tax-exemption or reseller certificate. For most states an exemption certificate will suffice and as a service we have linked below a blank Multijurisdictional Form. If your state does not assess sales/use tax, please provide a copy of a business license, vendor permit, or the like.

​​​​​​​Until you complete the below AND we have updated your account to be tax-exempt, you will initially be charged any applicable tax with your C2G orders.

If you already have the required forms and documentation completed, you may submit them and apply for tax exemption at the same time as your account creation. You can also apply for tax-exemption at any time by visiting your account page.

Check For Additional State Requirements

  • Florida - Copy of your signed Annual Resale Certificate AND the completed Multijurisdictional Form
  • Indiana - Completed ST105 or the SSTGBform F0003
  • Louisiana - Copy of your state-issued R-1064
  • Massachusetts - Completed ST-4 or ST-12
  • Mississippi - Copy of your state-issued Sales Tax Permit
  • New Mexico - Copy of your Non-Taxable Transaction Certificate executed by the State AND the completed Multijurisdictional Form
  • New York - Completed ST-120 or ST-121
  • Virginia - Completed ST-10 or ST-11
  • West Virginia - Completed SSTGB or F0003
  • Wyoming - Completed SSTGB or F0003
